I feel this way even when I'm alone. I'm not sure what to do anymore. Sometimes it prevents me from writing and becomes an overwhelming paranoia. Even though I know that nobody is watching me, I can't help it. It happens automatically, like there's someone waiting behind me judging me

Ernesto Sabato's On Heroes and Tombs is a very good book, there's a schizo that reads Hegel and thinks that all the homeless and blind people are organized and have a network for controlling things and that he's being spied by them because he knows this. The third part of the book is a very trippy schizo account on encountering a subterranean world. He is just one of many characters though so you will have to get through normal
stuff about love and jealousy.
