It's not the first big work of Japanese literature. Most literature before that was written in Classical Chinese or in Man'yogana, and there is a lot of literature from around that time that isn't nearly as repulsive in its themes.
Also, Genji was generally regarded as "lascivious" by Japanese Confucians etc until the nineteenth century. It was not nearly as prominent before as it is now.
