Anhedonia is a kind of withdrawal. Arguably you can recover without pharmaceuticals but If you had to come off due to political instability it would also be a really bad time to be coming off.

Not true. Regaining HPTA function after long term steroid use takes 6 months to 1.5 years using a mix of different drugs including HCG, HMG, FSH and clomid. This is not practical if you need to come off quickly. Many people continue to take HCG or TRT for life to maintain reasonable vitality and resilience. It's already hard to get all these compounds and would be even harder in times of political instability.

I don't think Spengler could have predicted the explosion in technology and communications that separates us from previous civilizations. (Though please correct me if he did as I have not read him). In my mind, complex, highly refined, high technology products along with fragile global supply chains creates a recipe for sudden collapse rather than slow decline. Or if not collapse sudden drop in supply for certain products (e.g steroids, or needles). We have already seen some of this during the current pandemic.
