In the game all of the girls have their own story branches, called routes. So there's a route where he ends up with Tomoyo, one with Kotomi, one with Kyou, etc. So After Story is the continuation of Nagisa's route. They put most of the story for the other girls into the anime but they removed the romance with anyone other than Nagisa. However, there's an extra episode of the anime that adapts Tomoyo's route, and one that adapts Kyou's route. Tomoyo After is a sequel VN to Tomoyo's route, so it starts with them already in a relationship since all of that is in Clannad.
