Pain in literature.

What's the most painful thing you've read described in a book? Can't think of anything solid off the top of my head so excuse me if I use something from a movie. Get this; a guy opens up his living victim's midsection and he starts pulling out his uh liver or something, and then he starts to crush it in front of the guy who is screaming. It was all a dream but uh, it still looked pretty horrific and painful
