How do you improve your reading speed? I've heard that the more you read the more efficient you become at reading. But, I read a substantial amount and I feel like I progress through a text at an average rate. Any suggestions for improving?

Publish three or four versions of the same story, with subtle differences. a paragraph here, some dialogue there. Never reveal how many different versions there are.
Make sure one of the versions has a limited publishing run

I just finished reading Lolita. I thoroughly enjoyed it, but I was bothered by one thing. On the final meeting between H.H and Lolita, the end of the chapter almost implied that he killed her:
"my American sweet immortal dead love; for she is dead and immortal if you are reading this. I mean, such is the formal agreement with the so-called authorities"
This follows just after he mentions the idea of brandishing his gun. I know this is not the case, yet Nabokov only explains the "authorities" in the final paragraph. So, was anybody else annoyed by this, or did I miss him mentioning the publishing agreement at an earlier time?
