If you get the references to Hell and other things like the name of the first chapter being April Fools and Paul Owen still being alive , yes he absolutely killed people. It may sound like an ironic point, but what you're thinking is not the twist. It's that all of existence is diabolus ex machina or Murphy's law. You have to throw your normal logic away for the story and understand that the things happening to him are there to trap him into a viscous cycle of desperately trying to feel something, even if that is punishment for his deeds, but when he starts to beg for it, he cannot get it. But because the universe is cruel, it cannot allow him to feel love, forgiveness, or any kind of release. It has to torment him and that is why he has to torment others. Also I believe Ellis did insist in an interview that Patrick did literally kill people in the story, and he also does so in cameos in other novels.
