You can write your own setting entirely from scratchy if you want it. If you don't take this route, however, you will need to weight the pros and cons of taking another setting. If you wish for a accurate portray in the story about the setting you choose, you will need to make a little bit of research. Some settings are easier to reasearch than others, but still, this is your choice. You can also make a setting without truly having certainty of anything. In the end, the story flow and the characters will probably be more important than the setting.
>lore
To create a lore for a ficticious society you need to take into account the setting and previous events, mythical or not. Real life lore is well documented for most cases, so if you don't make something from 0, again, research. And remember that the setting defines the lore. The interpretation of such lore changes in every setting.
>time period
Well this on the ficticious universe will be a question of the ficticious universe itself. Is it the start of a civilization or a late stage one? If you choose a historical setting, than, again, research.
>themes
The themes will probably be limited to all the previous topics I already talked about. In a historical setting, you can find themes that were of interest of the people of the time and place then other themes, with will help you in your plot construction (for example, I wanna make a story on X place in X time where Y subject isen't as important than Z. How can I connect Y to Z? Or you could make a dude going after Y, even tho the world say for him to go after Z, with would be a story of taking your own actions in the end).
>even characters
The characters can a bit tricky. Tho if you make the all the above, it can greatly help you to get into the mindset of a character that lives that reality.

Why don't you make a urban fantasy/sci-fi about muder mystery? The cool point of making your own universe, is that you can create really interesting combinations.
Why don't you portray the sci-fi setting in such a way that the technology allows people to do things that will be considered magic? This is a very famous concept (the one that tech reachs a point so advanced that is amoust just like magic).

Maybe you could start with inspiration in something. I took inspiration from some books I read, even the more technical ones. You can also find inspiration in your dreams. Recently I had a amoust southern gothic nightmare I already have one idea of a setting I could develop.
Don't push yourself too. Creating a working universe that isen't broken in itself is hard, but in the end, it can turn into truly great experiences, both for the reader and the writer, the last one because it helps to develop their imagination and world building.

I probably won't follow any medical path, but I am very thankfull for all the tips and commentarys I got. I have this books for more then a year now and only today it crossed my mind to ask /lit/ about it. I a glad I had all these responses. I also can share the feel of people thinking you are super smart because you are studing a more classical subject in the academic world (I have 2 years of studing law and I can tell the difference of people's reaction when I said to them I was studing law them to the reaction they do when I say I am studing history currently) but still, I gave much respect to the people from more mathematical fields like medicine and so on. I hate math, I realy do, I wished I didn't hated as much and was better at but still, I can't help it sometimes, even tho math is such a basic knowledge.
