I'm sure there are more than one way of doing it, but what I try to do (depending on the book) is after every paragraph ask myself how it relates to the books main theme and it's subplots or it's arguments. And I try to answer it or acknowledge that I don't and move on.

When it comes to understanding a paragraph I try to understand what a word refers to rather than what it's dictionary definition is. If a book says "it was awful" I need to know what "it" refers to and I need to acknowledge it. If I don't know what it means I might as well have read "something was awful" which becomes a meaningless statement. Like that you have to go through the entire book. Each statement in a book has a meaning beyond it's words and those need to be recognised. The people who say they read 500 wpm are stupid and rarely know what they've read.
