Plato describes Socrates as a doctor of the soul in the Gorgias. That is, investigating reality through metaphysics in the search after truth is perfective of the soul in a way analogous to the way in which medicine, as practiced by a physician is perfective of the body.

So, both make sense—but it depends on the reason why the person is doing metaphysics. If they only care about sublime knowledge and not the state of their, or anyone else's, soul, then they are a metaphysicist. If, on the other hand, they care about the care of the soul in coordination with sublime knowledge, then they are a metaphysician.

Most philosophers today are the former at best.
