I buy all of my books second-hand and will spend a little bit extra to get a copy with a cover that I prefer. Sometimes it doesn't work out but that's fine and I don't mind, because it's just a book.
But yes, I think anyone who claims that the cover doesn't have any impact on their reading of a book is in denial, particularly when it comes to fantasy. Pic related is the edition of Elric of Melniboné that I own and the cover suggests nothing of the actual contents - the book is nowhere near as dreamy, surreal or Romanesque. The guy on the cover doesn't even resemble Elric, only the helm bears relevance. A little bit of visual info goes a long way in catalysing the imagery that the book is attempting to delineate.
Also if I'm going to be spending money on something that is going to sit there after I'm finished reading it, then I want it to look somewhat appealing.
