Because Tolkien codified the generic fantasy setting by inspiring an untold number of copycats. Tolkien's shadow is so incredibly long that even motherfucking Japan defaults to faux European middle ages rather than their own equivalent period. At some point the medieval fantasy setting gained a momentum of its own. Don't underestimate the power of familiarity. It's why brands work and corporations blow obscene amounts of money on marketing. People are so familiar with medieval fantasy that the setting is comforting by itself. And it's self-sustaining. The fantasy writers of tomorrow will recruit themselves mostly from the fantasy readers of today. Being so very familiar with the ubiquitous medieval fantasy novels inevitably makes them more comfortable to write another one for the pile themselves instead of taking a less traveled path.
