At the level where there's a single leech caterpillar right after the checkpoint, with a pit leading to a cave between it and a dog. To the right of the dog there's a pitfall with a vein at the bottom. Get the dog to jump in there, then jump into the vein, die, wait for the dog to start raping you, then select 'do not continue'.

Yeah, that's what I thought. But after changing it the only new ending I got was with the mosquito. I lost to the spider a couple of times to see if there was a CG scene for it, but I just got the usual CG scene with her on the ground.
