I do it by character if I have enough images of a single character. Otherwise I do it by game they show up in with an extra folder for group shots.

I personally do it manually. Sorting by character or pairing of characters. If there is a lot more characters than a specific pairing I usually stick to the game most are from or just generalize it. Also keep lewd pictures away from normal pictures and wallpapers away from the rest.
