100% or close to 100% male games/manga/anime can be pretty fucking awesome if done right. Examples are Saint Seiya or JoJo or anything by Nobuyuki Fukumoto.
But part of the charm of Touhou is that they're all little girls. It would be too different.

Touhou is a very generic shmup with good music and good characters. If ZUN had replaced the sprites with space ships before release, it would just be seen as another fun shmup with a great soundtrack like Ginga Fukei Densetsu Sapphire or the various CAVE shooters. Plenty of 4channers would play and enjoy it, but I would imagine very few people would consider themselves "fans" as so many do with Touhou.
