A window that covers my entire screen, which makes alt-tabbing easier. If it's not available, windowed.
However in FPS games, fullscreen is mandatory forme.

Pretty much this. I've mostly got chat windows open and whatnot, so being able to alt-tab between windows quickly is nice.
Usually at a resolution that fills up most of the screen.

I actually can't even run some of the earlier Touhou games at fullscreen on my monitor. It's too big or something. The screen just blacks out...

I have a 1080 screen, so playing fullscreen warps a lot of things to unwatchable proportions. Windowed is almost always better anyway to see the graphics at the resolution they were meant to be.
