They do admit shiki came from a parallel world (that she gets sent back to after being defeated) so maybe shiki has been on a cursed journey through alternate type-moon realities hunting servants. Some worlds probably only have the usual 7 at any one time, other worlds like this one have more. When she kills 100 she is freed from the curse.

So it's likely the other 99 she killed didn't come from this particular reality.

The girl with the white dress is a master in this game, she calls herself Alice because she's a big fan of Alice in Wonderland. The other girl is a Servant, she represents picturebooks, which achieved legendary status because they brought hope to millions of children. The picturebook has E in all stats but has A and A+ in things like Metamorphosis and World Creation, giving it the power to assume the form of whatever his master wants. in this case, a friend to play games with. in reality the master is a coma and about to die because she suffered some sort of horrible injury when she was little, and what the master you encounter in fate / extra is just a ghost-like manifestation of her.
