I'd recommend any of the following...I don't 1CC lunatic or anything like that but as your first Touhou this is how I'd feel about them.

Embodiment of Scarlet Devil: Good. Not too terribly difficult, but some of the mechanics are not as "new player friendly" as in later games. Good characters but you might wonder why everyone is trying to eat Reimu.

Imperishable Night: Easiest Touhou. Up to 7 lives/3 bombs? per continue and continue where you left off rather than at the beginning of a stage. Continues are very limited if you intend to make it to the end though. Lots of variety in character selection.

Mountain of Faith: My favorite, very midrange difficulty and good characters, music.

Subterranean Animism: Higher difficulty than most, but good characters and music. Most rewarding easy mode (you can 1cc on easy to see the good endings) if you're going to be doing that. On the negative side, all of the option types suck dick, or at least feel much weaker than the ones in other games in the series.

Ten Desires: Easy and straightforward. It was designed to be simpler than most, so not a bad first stepping point.

How could you possibly skip Perfect Cherry Blossom? In my opinion it's the best one to start with. The easy level difficulty is reasonable, normal isn't terrible, and the mechanics are reasonably simple. I also feel the games should be played at least roughly in order, but I wouldn't feel right about introducing someone to the series with EoSD, so PCB is the ideal start.
