The radius of effect of a nuclear explosion may seem big, but the outer regions are where people suffer minor structure damage, bricks falling from roofs, etc. Most of the effect of the outer radius is radiation / heat burns. In modern towns/cities the concrete buildings would still mostly remain in tact. There is no reason to use nuclear weapons unless your aim is to murder a lot of people. Tactical non-nuclear warheads can destroy specific targets, where nukes are overkill.
