I expect this depicts one of the countless inevitable and inescapable bad ends, althoug hI do wonder if those wounds were inflicted by Homura-chan, even though Homura-chan cares for her very much. It may be that this is a mercy killing in one timeline just before Madoka would turn into a witch and destroy the human world.. I don't know what else it could be.
