Well, the country and environment is really the key factor in how rough and open bullying is.

Most of the time the bullying is ambiguous and not heavy physical, because that leaves evidence. Bruises, scrapes, etc are normal, but broken bones and stuff are usually caused 'accidentally', like pushing down the stairs, contact in sports, and other things that people can easy find other blame for. Emotional and Mental bullying is far more common in a high school setting.

One thing that some cultures have is a very strong group mentality combined with a warped sense of shame. This leads to automatic segregation and being a victim is shame being enforced by the society as a whole. Also in those cultures, if the 'bully' isn't acknowledged as a bully by the class or school group, they automatically think what they are doing is fine.
Look up the shame culture if you are more interested.
