She accepted her condition and decided to enjoy her life as much as possible since she doesn't know how much time she has left, thus she forces herself to be the genki girl.

Alternative idea:
Her poor circulation resulted in a stroke at some point, which caused brain damage in her auditory lobe.

Every possible disability can be applied to having a weak heart. I'm only mentioning the deafness because someone spoke about it earlier.

Misha is riding Hisui and suffers a fatal heart attack. She slumps over and smothers him with her fat fat fatness. He tries to push her off, but is too worn out from the minutes sex. In his frantic attempt to not be suffocated by his blubbery lover, he induces his own cardiac arrest and dies.

Lilly enters the room trying to find Hisui, but doesn't see them. She trips over the bodies and loses her guiding stick. In terror she calls out for help, but Shizune can't hear her from the next room over. Emi and Rin find them but freak out. Emi runs of to find help but slips down the stairs, while Rin stays behind to lend a hand to Lilly.

And during the whole time, Hanako is in the library reading "The Aristocrats."
