The callers voice was described like an odd boy's voice, if you lose your testes your voicebox never changes while the rest of your body get larger.

As for girl losing her genitals from a fall, its plausible but it doesn't seem to sync quite as well with the gender identity issue hovering over the story (girl with a disfigured vagina being raised a girl vs a boy with a disfigured penis being raised a girl).
