From the looks of it, they are standin on top of the pyramid that Souther forced Children to build.

Not only that, but if you look at her left leg she appears to be peeing.

What's more they seem to be on top of the pyramid where the central stone is.

Souther forced the blind warrior or justice shuu to carry the central stone all the way up the pyramid, under the threat of killing all the children if he failed. Souther cut all the tendons in Shuu's legs too.

Crippled, Shuu carried the stone all the way to the top, where he was impaled by Souther with a thrown spear.
At this time Kenshiro arrived and ran to the top, arriving just in time to hear Shuu's last words and to watch Shuu be crushed under the weight the stone.

So it would be my guess that in that picture Souther is making Sakuya pee on the stone as a insult to Shuu and/or Kenshiro.
