Japan have a long fine tradition of treating handicapped/crippled very poorly. They are considered nothing more than an inconvenience and a disgrace to their families and lineage. I'm sure you heard the common statement in anime/mange someone doesn't want to "be a burden".
As non-conforming, parasitic, sub-human elements they're expected to at least at least stay out of sight if they lack the insight and strength to "remove" themselves by suicide. And many didn't even get to decide even that for themselves.

To this day it is still not uncommon for cases of families keeping members locked up for decades without anyone claiming to have noticed surfacing. Japan didn't buddy up with nazi germany and ideology by coincidence.

They say you can judge a society on how it treats its weakest, if that is the case Japan still have a very very long way to go. Not saying there are places still, if not worse, in the world but these are mostly poor underdeveloped shitty 3rd world countries.
