I haven't played it but saw a few clips of the gameplay on youtube. It looks like just another shootan with girls and japanese folklore "plot" and music.

Well OP, in all honesty that's the wrong way to do it. Much in the same way that there are some games out there that are more fun to watch than to play, Touhou is one of those games that are more fun to play than to watch. Of course, if you understand what's going on throughout the video, you can undoubtedly appreciate some aspects of it that a non-touhou game such as yourself cannot, but by and large touhou is something that provides the best experience when you're sitting there, controller in hand, listening to midi music blasting synth-piano and trumpets at you while you look on in wide-eyed terror at the waves of bullets careening towards you

If you're not interested in it, then that's fine. But do give it a try yourself before dismissing it!

Touhou is cute, challenging, colorful and it has fuckloads of lolis flying around and shooting lazers. Perfect game. And music. Did i mention the lolis?

It's challenging yet "honest", you know you could beat it if you had better reflexes, not much fake difficulty, expecially considering the genre.
The music is just great.
The dialogues are funny.

Intricate bullet patterns and a perfect balance of difficulty for anyone not in diapers generally hold people's fancy for long enough to produce something worthwhile. As to what started it, obviously ZUN's talent as a composer and his interesting character design (but lack of artistic ability).
