Suwako is named after the lake, but she is a "Goddess of Mountains," according to ZUN. I thought for a while she was a fertility goddess, which would also make it weird about the lack of breastiness, but that's just the Mishakuji that she beat and controlled.

And it could've been worse. Look at the Egyptian frog deity, Heket, instead.

During ancient times, she was a god in control of "Mishaguji". Mishaguji were curse gods who had obtained enormous amounts of faith, cursing birth, harvest, military, and many other things. Making light of these fearsome gods would lead to divine punishment. The only one able to tame these gods was Suwako. Because of this, she gained an extraordinary amount of faith, and cultivated a kingdom as its ruler while still being a god.

In Touhou or the real world? In Touhou, the Mishaguji were a group of curse gods who cursed and ravaged the people in their lands. They gained a large amount of faith through fear. Suwako tamed them, gaining a large amount of faith for saving the land and the people. Etc, etc.

TLDR: Curse gods tamed by Suwako. They're the ghostly whites snakes that Suwako uses as attacks.

Native god worshipped in Suwa region. He was a fertility deity in form of a penile earthworm of sorts. Moreya was a dominant clan in the area back then. Then Suwa absorbed Izumo and the dominant Izumo deity Takeminakata (as well as his wife Yasakatome) sort of hijacked his spot. Mishaguji is currently enshrined in Misayama Shrine.

Very few mentions of the fellow in English, patched together what I could find.

Mishaguji was a native god commonly depicted as an earthworm or a snake.

Associated with the Shaguji (御石神) stone deities, he was said to dwell underneath rocks and boulders. His name can be written as 御赤口神 (red-mouthed god), or 社宮神 (god of the shrine.)

He was a harvest god as well as a fertility deity, and it's said that offering a prayer to him can make sex a more enjoyable experience.

Once the Yamato took over Japan, Mishaguji was seen as vulgar and and became known as an evil curse god, and it became taboo to worship him. He was mostly replaced afterward by Moreya-no-Kami and Takeminakata and is now pretty much made obsolete by those two.
