People do show up to her shrine to pray/worship. It just doesnt happen nearly enough because of how lazy and stupid she is. For example there are lots of moments in WaHH where she initially gathers a whole bunch of villagers by enticing them and has some success but eventually loses them all only for this to happen again sometime later. So it's not like her shrine is a perpetual ghost town. I can imagine she has less than a couple people show up every so often. Maybe some random dude wants to heal his sick grandma but doesnt trust the aliens in the bamboo forest so he goes to Reimu for prayer. Or someone dealing with a stupid yokai.
