You saved it as a .jpg, which will smush out the effect in it's compression and break it, that's why you can kinda see both images overlayed on one another.
Use .png instead, or any other lossless format that keeps you in the filesize limit.

yep, but since 256 colours is the same as what a 90's computer could handle, you'd be surprised at how quickly you run out. Hence the posterisation and general colour banding as everything gets rounded to the closest thing available.
Technically of course, this is lossless, but since that overlooks the huge palette limitation, that's only technically correct and therefore practically incorrect.
