Usually it refers to a fictional character you love. Loads of people use it like ''this character is soooo cute, she's mai waifu'' but the real meaning would be a character you LOVE and will love for a long time.
Some people think idols etc. can be ''waifus'' as well, but a lot of people probably only accept 2D waifus. In my opinion idols can be waifus, too, because in the end they're as unreal as completely fictional characters.
Oh, and it would probably be better to think they must be from a Japanese form of media (anime, manga, video games, VN:s...) to be a ''waifu''.

A waifu is, generally, a girl that does not exist outside of mediums of fiction that a person becomes attached to. Also, at the risk of plagiarizing Harry Potter, one does not really choose a waifu as much as a waifu chooses them. That is, you don't really decide, "I want her to be mai waifu". Rather, you just go along your daily life until you see/read a girl that makes your heart flutter. I believe one man got it right when he described his waifu at first as "She was my every desire in a girl." Everything about her, even the bad qualities, bad either to the general populace or to other otaku, does nothing but increase her attractiveness to you.

As for what one 'does' once one has a waifu, I would say that it depends on the individual. Some go out and download every picture of her that exists, regardless of the space it takes up, and then go to check danbooro, gelbooro, pixiv, etc weekly or monthly for new pictures. Some buy tons of merchandise about their waifu and decorate their room with her so that they can always see her. Regardless of how one accumulates her physical image, or if one even does such a thing at all, most people with waifus imbibe themselves with fantasies of their waifu. Whether it be day-dreams of the otaku going on a date to the beach with their waifu, or just sitting at home together, cuddling in bed, 'the waifu' is used as a means to get happiness, to add some joy to their otherwise dreary life, and, in some cases, to add the component of a companion to an otherwise solitary life.

I think it could be. After all, just as Romeo was in love with the idea of being in love (before meeting Juliet, at least), and not really Rosaline, or any other whore he chased after, so, too, can one be in love with the idea of a waifu. It could be one's ideal of the perfect woman that one is in love with, or it could be simply the idea of giving one's love to a woman that does not exist.
