Two of my friends lived in Japan for five months and their Japanese is still laughably bad. Being surrounded by Japanese helps, but you have to make an actual effort to learn.

An elementary textbook to get started (Genki I&II are good and cheap, in fact you can torrent them last I checked) make sure to follow proper stroke orders, and make sure you master this textbook and can read all the conversation bits smoothly and naturally.

media etc..
Everything you've been reading, watching, and playing up until now but raw, listen to lots and lots of it, some have recommended listening to speech (say, a news stream) while they sleep.
Try Skype for speech practice.

Prepare to read simple 4koma manga one panel at a time one voice bubble at a time while you look up every word/kanji you don't know, it's slow and excruciating, but through repetition, you do learn them.
If you have a DS, I recommend the 漢字そのまま楽引き辞典 dictionary rather than dumping a bunch of money on one of those fancy electronic dictionaries.
There are also functional iphone dictionaries which seem pretty handy but I haven't tried one.
