If it makes you feel any better, here's a little horror story that I've experienced.

The first time I ever played Phantasmagoria of Dim Dream, taking god knows how long to beat it, and just barely beating the final boss by like one single life.

When I got back to the title screen, I went to Options to see what difficulty I was playing on. It turns out, I was playing on easy mode the entire time. Oh god the horror!
