Never. She's some weird sick girl who can barely breathe. I'm fairly sure she's a skeleton below her robe.
And not sure if youkai can even fatten up like humans can so you'd have to search among those. Otherwise it'd clearly be Yuyu since she eats by far the most from all of Gensokyo.

Smelliest ass: Meiling, after a heavy, sweaty training session and some spicy Chinese food resulting in powerful, gusty braps.

Smelliest vagina: definitely Sakuya. She honestly has no idea she's even supposed to be washing that thing, and has never done so in her entire life. There are unknown insects crawling around in the thick matted wads of pubic hair clumped together with dried period blood and soaked in piss.

Smelliest feet: the Remi. Five long centuries without a single wash. Armies of hopeless foot slaves addicted to huffing her pungent cheese-encrusted foot fumes laced with powerful fungal spores that subvert the nervous system and force her slaves to keep coming back for more.

An SDM hat-trick!

Honourable mention: Sanae, after a day of heavy use by hundreds of her oldest, most loyal, and least hygienic customers can make the whole of the youkai mountain smell like an unwashed whorehouse mattress if the wind blows in a particular direction. Sanae really prefers to wash regularly, it's just that her most devoted customers prefer the more 'powerful' olfactory experiences she provides.

Akyuu often doesn't wipe properly after taking a dump, but no one cares about her.
