No, Gensokyo is physically a part of japan, merely separated by a barrier. Also Reimu's shrine exists on the border apparently, so she can probably go from one side to the other at will.

Besides her, there's Sanae and probably Yukari because I think she's older than it.

Hakugyokuro is technically outside Gensokyo since it is situated in the Netherworld

Komachi and Shiki aren't in Gensokyo due to their positions. Komachi being a shinigami on the Sanzu no Kawa and Shiki being an Enma in Higan.

Eirin, Kaguya, and Reisen are from the Moon.

Scarlet Devil Mansion originated in the outside world but was moved to Gensokyo.

Moriya Shrine along with the lake and everyone who is a part of the shrine was also moved to Gensokyo from the outside world.

Maribel and Renko are canonically the only two characters who are not in Gensokyo. They're both university students in the outside world.

You know, this suddenly explains a whole lot. Touhou is just a moe representation of Japan's inherent racism. All the bad guys are outsiders, while Reimu and Marisa are Gensokyo born and bred.

We don't really know that since the "Lunar Wars" could just be the mere presence of the astronauts and their technology fucking with the moon's barriers and magical beings or whatever. Someone once theorised that each lunar landing could be the equivalent of a nuke being set off for the lunarians but it's nothing more than an idea. It's all valid though since most of touhou is the shit fans make up.

OFFICIALLY THIS IS WHAT THE LUNAR WARS WERE:

If it were an outside, human influence, like in the last Lunar War, it wouldn't be a problem. Long ago, the humans had made it to the moon's surface and claimed it as their own by planting a flag. They were probably impressed by their own science and thought the moon really was theirs.

But, if you know where to look, the moon's technological prowess was far beyond that of the Earth's. The humans had spoken of building a base on the moon, but in the end they ran back home before ever deciding on a place or even acquiring the means to do such a thing. It was a terrible defeat for the humans.

The humans' lunar landings were reported as a great success in the outside world, but their failures were not. The humans kept losing after their first landing, so eventually they stopped returning to the moon. And we, who'd remained in contact with the moon, knew how every time they tried to establish a base, they'd failed.
