Bern is just Rika's soul aged over all the repeats, Bern acts as an adult because Rika should be an adult by now (not to mention dead)
when rika realised this, she decided to name her older self by looking at her wine bottle, and it said Bernkastel. so, ta-dah.

Think of it like files in a computer. There are millions of computers named Rika, each with an internet link to a different world. Hanyuu has a portable hard drive that she connects to her Rika. When Rika dies, the files are dumped into the portable hard drive. Hanyuu takes the data from that portable hard drive, hooks it up to the next Rika, and overwrites that Rika's files with the ones from the portable hard drive. Eventually, the portable hard drive has accumulated an extremely large number of files. When Rika finally succeeds, she continues living happily on that computer. However, the portable hard drive eventually becomes Bernkastel.
