If it reverses cause and effect, why does Gae Bolg need to be anywhere to be used?

Can't Lancer just be chilling in Ireland, say "Gae Bolg", and Saber's heart is pierced? Gae Bolg'll find its way there eventually. It was already pierced, after all.

In the Fate route, does Gae Bolg actually loose it's power to always hit once it misses Saber's heart in that fight at the beginning? I believe the text said something like Saber had 'sullied it's name' by being able to avoid being killed by it.

Probably yes(concerning the causeality effect)
Best Bet for Lancer to defeat Berserker is to use his stat-boosting runes to kill him with a normal attacka few times (hit his vitals), the causality release, and the thrown attack and claim more than one life each time.
Bad odds, no matter what.
