I'm assuming you already know this, but just to clarify...

The soldiers that show up in the river are the exact soldiers you've killed throughout the game, down to their individual injuries. If you don't kill any of the small fry, the only people you meet in the river are the Cobras (who appear regardless of whether you actually killed them or just stunned them). So yes, it can be argued Big Boss did kill the Cobras, or as the river is a reflection of his conscience, that he at least feels responsible for their deaths.

The Cobras, plus the Boss and minus the Sorrow, are still "only" five people, though. In stark contrast to the younger Snake who kills people left and right.
