It was not really "anemia". He didn't lack blood. It's just that because of his eyes he was less attached to his body than normal people. He rejected death. Nanaya, even with mystic eyes, didn't suffer headaches and didn't have anemia. I'm pretty sure as satsujinki he did experience exhaustion from being faster than hios body can handle, but didn't have any problems staying in the forest traveling for a few days.

Ren's route. Shiki vs. Shiki.

His "anemia" is caused by having his lifeforce stolen by SHIKI and sharing half of Akiha's. When SHIKI is inactive, he's fine. When SHIKI is active, Shiki suffers attacks. His body is perfectly fine and is almost inhumanly agile.

In Kohaku's route, his headache stops because he accepts his killing urge, which was causing his head to split. The focus and drive he has keeps his mind from suffering as quickly as he normally would. The Nanaya Nightmare didn't have Eyes of Death, just the jougan, which should've let him see unnatural things. Why he missed seeing the Kouma Nightmare, I don't know.

In every route, regardless of the outcome, Shiki will die young.
