I think they're immortal as soon as they have the elixir, not from having it perpetually in their system. Otherwise, they wouldn't really be immortal since there are plenty of ways of killing them that would involve draining them of fluids. In other words, the elixir affects the soul, not the body.

Granted, Arc didn't die when she was killed, but she did have to rebuild her body from scratch. At worst, Shiki can't kill Kaguya, but he can turn her into helpless chunks of immobile flesh for the rest of eternity. Fun stuff.
