There is a real world equivalent to every Touhou character. Here we see Reimu's and Yukari's doppelgänger, Renko and Maribel.

I never knew a bamboo forest at night could be such a mystifying place! There's no signal for my cell phone here, nor does the GPS work, and there are rare natural bamboo shoots for the taking.... Maybe I'll rest around here for today...but I'm actually in a dream, am I? Can't be helped, I guess. I'll walk around a little bit and see.

And then there is the sky filled with stars. Whether it is the underdevelopment or the clear skies, this place seems like Japan a long time ago. A time slip, perhaps? Was Hawking's reversibility of the arrow of time actually true? If there weren't monsters around here then this place would be more fun.

That's right, could it be that the dream world is actually the memories of the substance that makes up the soul? Monsters are the symbols of traumatic memories. Hmm, that's new. I'm gonna tell Renko about it when I wake up. Now then, maybe I should start wandering again.

- This was discovered several hundred years ago in the Bamboo Forest of Lost. There are many vocabularies with unknown meanings, so it is still undecipherable. Though it is thought to be written by someone from the outside world, what, really, is the meaning of "dream world"?

A interesting thing that doesn't involve Maribel but gives an outside an idea of how Gensokyo works is some things that Rinnosuke says in Curiosities. From Chapter 10 and 11 respectively.

" At this point, I got to thinking about how the barrier that surrounded Gensokyo affected things. What the barrier affected was people’s “thoughts”. If a material wall would be a “wall that keeps the body from passing through”, then the barrier would be a “wall that keeps people’s thoughts from passing through”. Going trough the barrier – in other words, to be spirited away – is something that happens when one is in a particular state of mind, where consciousness is hazy, and then your whole self would jump over it. For only an arm to jump through the barrier, it would mean the arm and the body have different thoughts. A human whose arm and body move with a different will? I don’t think there is such a human, much less a lot of those. So maybe what Reimu said was right, and this is not a human arm. "

And Chapter 11 -

I grabbed the small, white metallic box, put it to my ear and closed my eyes. Maybe I could hear some sounds from the outside. I got the impression that there were some conversating voices coming from outside. Maybe Marisa had already came back, or else a youkai had come by, lured by the fried tofu… No, I don’t think that’s it. There was an ear-shattering clamor like nothing I’d ever heard before. I couldn’t believe such a painful sound could be made by anything living creature.

I felt unpleasantly warm air all over my skin. It was as if the ambient temperature had changed all of a sudden. If winter is to be like this, I don’t think I will need heating. Even with my eyes closed, a flood of light is pushing through. What could be shining so bright like that? It's a cold brightness, unlike sunlight or magic light.

Then I had a flash of intuition. Right now… I must be in the outside world. Riding on the outside items surrounding me, my thoughts had flown across the barrier. But... I didn’t open my eyes. What if, after seeing the outside world, I couldn’t return to Gensokyo anymore? Humans that are spirited away almost never have a chance to come back again. On the other hand, if I open my eyes believing this to be only a visual and auditory illusion, my thoughts wouldn’t cross the border and would be back to Gensokyo, and I could miss my chance to see the outside world. Which is what I truly wish for?

That’s right, wasn’t my goal to obtain fuel? I did have a clear objective. I didn’t want to get lost in the outside world, only to visit it to complete a task. I had to leave my thoughts at Kourindou—no, at Gensokyo—and let only my body cross over there. Yes, that’s a feat humans couldn’t pull off… but I’m sure I could. For the sake of getting fuel for making my shop warm again, I slowly opened my eyes.

It was a flood of light. It was a very bright light, but also cold. I could hardly see in that brightness. And there were some voices speaking in what didn’t seem to be Japanese. The hot, foul air was making my head hurt. The outside world…I had seen it in things like books that drifted in, but I never imagined it would be so noisy and unpleasant. I had to calm down and look for some fuel, and then I should calmly look for a way to go back to Gensokyo. ...my eyes were getting accustomed. This place, this archway was something I recognized… is this a shrine? And a shrine with a big crowd in it…

“Oh, no! You came all the way here... You can’t come here. Because you are not human, you see?”

“!?”

That noisy sound suddenly stopped. The light also went away, and I was with the white case in my hand again. The surroundings were gloomy, but somehow I could see them… it was the usual interior of the Kourindou.

My theory is that one cannot cross the border into Gensokyo physically but only mentally, perhaps through dreams. Maribel with her border abilities is able to achieve this much easier, possibly at will while others ended up there randomly, at least as far as humans go. Youkai and other assorted magical beings seem to be able to enter it at will although I'm not sure if they can leave as easily.

Gensokyo itself seems tied to forgotten or things missing things also, like it has a copy of Mount Yatsugatake when it was supposedly taller than Mt.Fuji while the actual Mount Yatsugatake still exists in the outside world ( I assume ). Old forgotten objects like the original gameboy end up there also so I think there's no predefined border in the real world that no one can see but rather it's more of a dimension things can kind of just slip through like when you drop something and can't find it even though you know where you dropped it.

Actually when I think about it I suppose you could end up falling into it physically, if foreigners fall in and have their cells with them I doubt they were sleeping in their bed with them although I know I've done that before.

That's a good possibility, Rinnosuke likes to over complicate and over describe things. Though Reimu and Marisa mention that they had to weaken the border to get Yukari's attention which could of allowed Rinnosuke to slip through for a bit.
