There actually is a special input device for people who can't use their limbs. If I could remember the name for it, or could find the one we have around here, I'd post a picture of it for you (don't have much use for it right now as we don't have anyone here that has a use for it, and voice to text software is far more practical even if we did). Normally, though, you just use voice to text software instead of special tools. However, if you want to use something else, there is a mouse like device that you can control using your mouth and you blow into it to "click".

If you're really dedicated, maybe you could use the OCZ NIA. I don't know how customizable it is, but it could be pretty awesome if you could control your whole keyboard with it.

I actually worked at a place developing eye-driven tools for handicapped people. You simply look where you want the mouse to be, and there it is. It also uses blinking for clicks.

Was a fun job, too bad they couldn't extend my employment.
