Obviously never. ZUN has pretty clearly designated the matter to be something the fanworks should explore and there is no way he would ever off such an important character considering he has never done it for even minor characters (Fortune Teller doesn't count since his whole role in FS was to die and build tension based on that later, plus he was around for a whole two chapters). Touhou can be dark at times but ZUN doesn't roll like that. Also it would completely nullify a major point of her character, being an ordinary magician and all and generally the most relatable of the playable characters.
Oh yeah, and not to mention the fact that there is not chance in hell she isn't excluded from the whole "Human villagers becoming youkai is a sin" rule. She no longer lives in the village and is already considered a superhuman by the villagers, so the ruining the balance justification doesn't work.

Actually, I've been having ideas of a game based around this. Marisa is gone and nowhere to found while incidents happen throughout the village and youkai become increasingly violent. So the heroines (Reimu, Alice, Patchouli, Youmu) go around to look for the source of that sudden surge in violence. And it turns out that Marisa is the final boss and she turned into a youkai several days ago, which made the youkai go rampant because the "rule" has been broken.
And after the fight, the game asks you to press X to exterminate Marisa, which, if you do, essentially unleashes a more powerful bomb of the heroine, leading the second bad end of each protag. (the first one being the inability to 1cc, obviously, in which case Marisa runs away and the youkai rampage inside the human village)
But there would be a "hidden" input (the game doesn't tell you about it) possible at that moment where you simply fly up to Marisa, triggering a "it doesn't matter if you're youkai or not, you're still our Marisa" hug ending, in which Marisa still decides to revert into a human (because blahablahblahYukariblahblahblah) because it just didn't suit her.
