Kotomine dies at the start of lancer route. In it, as soon as Lancer runs away from Saber, you realize that its in the direction of the church on the hill. You missed that choice way back. When Shinji tearfully admits to you that he's not really Rider's master you're supposed to comfort him about not being a magus and his love for Shirou comes out. Just make sure not to let Zouken escape or he uses a crest worm on Shinji and he comes to your house and you get bad end anal rape instead of a tender scene.
