Is it worth playing through this again with the Realta Nua voice patch installed? Is the voice acting really that good?
If it is worth it, is there any way to install the voice files without reinstalling the entire game?

It is. For Fate/UBW only pay attention to the battle scene's. Since in terms of impact,thats all the voices really add for those routes. In Heaven's Feel,you should do the whole route over again(Skipping the I AM THE BONE OF MY BREAKFAST parts of course) with the voices,beginning to end. It really adds alot to the experience.

One more thing,do not forget to redo the bad end where Shirou chooses to not be Sabers master and gets mauled by Beserker. His screams from that scene are just so horrificly orgasmic.

I found the voice patch to be very helpful and add a lot to the experience. It added a lot of character to Sakura, Rin, and (oddly) Taiga. I was a little bit disappointed with Saber's lines though. She seems pretty monotone throughout the game.
