You end up giving her a reason to live and to struggle through her troubles.

You become a point of light in the pitch black dark that was her life.

When she eventually beats brain cancer through the power of love (Yes, you're that awesome) your world and borrowed time fades away. Memories of you disappear and you're left with an epilogue of a girl leading a wonderful life full of hope and desires.

You sit there at the monitor crying little bitch tears realizing that although you never got the girl you felt like you really accomplished something and to philosophically begin to break down the plot which results in you finding something 'deep'. You wash up, prepare your rusty social skills and walk out of your basement out into the world a new man.

What about this: a ghost girl who can only interact with the physical world by possessing someone. You scheme with her to get her the body she wants, incidentally a classmate who has a slight crush on you. You start dating with that girl with the actual intention of leading her to a quiet place, knocking her out and performing a black magic ritual to have your actual phantom girlfriend take over her body.

Good End: You succeed. Utterly devoted ghost girlfriend in your hot classmate's body, serving you in every way she can.
Bad End: You fall in love with the girl you intend to offer your ghost girlfriend, who goes batshit insane (well, she's obviously yandere) after learning this. You two stop her somehow, but at the cost of your classmate's life.
True End: The ghost girl never existed, turns out you were nuts. You come to yourself in a basement lit by red candles, laying inside a giant pentagram with the dead body of your classmate.
