The closest thing I can think of is Gore Screaming Show. Aside from the final route, you are pursuing a romance with one of four girls. Eventually, things get shot to hell, and a yandere loli from your past intent on killing your new girlfriend does everything in her power to ruin your lives.

Rena and Shion are both Yangires and not deres, Rena has already been explained. Shion can't be yandere because if her love of Satoshi was the cause of her psychosis then she would have entered kill mode in EVERY SINGLE arc considering she loved him in every world. Cooks, cleans, can tell if your food has been poisoned merely from the wight to a cherry tomato, give her a tiny bit of praise and she can block multiple thrown knives like it was nothing. Shion goes L5 due thinks her family killed him.This happens when hears Mion and Onibaba talking about Tomitake and Takano (I think), in Watanagashi-hen and Meakashi-hen (2nd/5th Arc).

She has his girlfriend (who she had been tormenting brutally for days besides this) and is having her tortured by her possessed mother while forcing him to watch. He's going kind of fucking crazy being made to watch this not able to do anything to stop it, and starts threatening Yuka that if she hurts Kiika anymore he'll hunt her down like a dog. She goes "Really? You'll chase me?" At which point I think he replies something along the lines of him never resting, I can't remember I'm probably making parts up here been ages since I've played. Yuka proceeds to have Kiika torn apart horrifically. As a result, he cracks. Shift to next scene, he's after Yuka in some abandoned factory (basically having become a broken shell seeking revenge exactly like Yoshiki) while she's laughing, smiling and asking him to keep chasing her always, forever and ever.
