It's a game about little girls and Shinto mythology in a medieval Japanese fantasy world. Japan is completely intractible from the setting. You can't have a much more oriental title than "[Oriental] Project" or [Oriental]. A 4kids style localization would render the series completely unrecognizable.

At most you'd see a localization strip out honorifics and use western name order, e.g. Reimu Hakurei instead of Hakurei Reimu.

Have you never seen Shaman King? 4Kids only changed a handful of names in that series, and all the Japanese stuff was left intact.
This got them a ton of complaints from parents, so they had to double down on the editing for their next project. You know, the one with pirates.

If somehow 4Kids were hired to make a Touhou localization aimed at kids, then they'd probably never bring up stuff like Miko's or Mamizou's surnames, but they'd still be called Miko and Mamizou (if the translator was feeling particularly creative they might use names like "Miko of the Many Whispers").
