Toss in an extra Personality, representing "The Gland" who merely wants to fuck anything, not sharing the views of killing as "That'll take away the chance of a kid to fuck in the future."
If a vote comes down to it, it'd rather disarm an enemy and restrain it to fuck it than just outright kill it.

Sides with Nanaya at times if the plan is Rape then Kill, just because theres a chance of the victim getting away before the killing.
Enjoys stalking women, even without intent (Well of course theres eventual intent.) to rape them then and there.
When it trys, it's rather good at wooing women, but due to the aggressiveness of the others, it can't ever get Tohno into a consensual relationship.

Possible route could be the sane personality working with "The Gland" to gain control over the mind.
It takes pity on the sane one just because it knows the sane one is trapped in there, completely useless.

Other potential ideas, are having Akiha's connection with Shiki allow her to exert her will into his mind at times as another personality who has a special vote in the democracy of Shiki's mind.
It usually sides with the sane side, but due to majority they always lose.

If the saner side works together with Akiha and The Gland's help, they can usually get things done.

Too bad all Akiha wants is to have sex with Shiki.
Which isn't really that bad a thing when you think about it.

Arc if she didn't supress herself to sleep wouldn't have been able to control true ancestor blood craving and would have ended up a demon god.

Kohaku would just have kept cracking and cracking until she found herself destroying or ruling a small part of the planet.

Hisui wanting to be like her sister would do it but in a more innocent and deadly way WITH HYPNOTISM!

Akiha would have kept drinking kohaku's blood and eventually inversion impulsed like SHIKI did.

Also all these characters are succeptible to inversion impulse making them a monster except probably arc.

Sacchin without the vampirization would have ended up taking shiki on a NICE BOAT ride. With the vamping she was already a monster enough (although retconned in melty blood to be quite sensible and a nice person still).
