Otaku is/was a negative term to descripe someone who is really into something so much that it's almost an obsession. The obsession part is what makes it seem unhealthy in the public mind. In the west otaku is mostly used in some way to describe someone who is into anime subculture a lot. Almost like an elite title. In japan there is also something of a otaku pride movement.
