The route went out of it's way to turn what you'd learned to expect from the last two on it's head.
Your innocent childhood friend who was believed to be completely normal and was kind of glossed over in the last two routes turned out to be a wormslut with a backdoor into the Grail system.
The main villain had barely been so much as hinted at in the last two routes.
Rider turned out to be legitimately badass after all, and the last Servant Fight of the last route is a mirror of the first real Servant Fight of the First Route, only with a different winner.
Servants dropped like flies early on, including Gil and Caster.
Saber is your enemy.
Shirou's character development went in the absolute opposite direction to where it had gone in the last two routes.
Illya had much more of a role.
Everything to do with Kirei.

The way F/SN works. Fate started relatively grounded for what it was, introduced the setting and rules things work on, UBW expanded on those and Heaven's feel just smashed everything, broke every rule and got insane.

Hmm, if I remember correctly, I felt like Illya had zero personality towards the end. "I'm not human, here's a review of some plot points, and now I'm off to sacrifice myself." So when she did, I felt nothing really. I mean, she knows better, right?

Everyone was suffering badly in HF. And Shirou had already risked everything saving her once before. And was still doing his utmost to save others.
And by the end of it Illya was grateful. But she knew that her time was running out regardless of everyone else's best efforts, and felt that it was finally time to do her part.

Illya often acts like spoiled brat and complete fuckwad. But she is still surprisingly mature underneath all that. And may act immature from the frustration of the poor lot she got in life. But have still had good time to think over and come to terms with serious topics. Such as her own and others' inevitable demise.

On an unrelated note, that last bad end with Sakura going senile alone was so heartbreaking even though nothing really happens in it.

Shirou was willing to give up his ideals for Sakura. He wouldn't for Rin though. All the more curious is that he saw what a loser he was going to be in the future.

The pure hax of the true end is very fun.

Shirou is now an immortal, transcendental soul with infinite prana, Sakura is still a conduit to a place filled with unlimited prana, and Rin has copied the Second Sorcery.

I like to compare it with the true end of UBW. The differences are jarring.

If it means anything, I prefer Realta Nua's H scene replacements in UBW and Fate.

Make sure to put them back in for Heaven's feel though.
